Looking for the perfect way to honor your child’s birthday and help the Murdock Elementary School Media Center?

Simply fill out the following information and send it to your child’s teacher, along with a check for $12, made out to the Murdock PTA.  Please write “Birthday Book Program” on the memo line. 

During a designated month, your child will be invited to a special reception for Birthday Book Program members, where your child can choose a book from a table, and a sticker will be placed in that book, saying it was donated by your child.  At a later date, your child can be the first student to check out that book.

 Why wait?  Enroll your child now, and you will be sent an invitation to remind you of the reception in your child's honor.  Note:  Birthday Book celebrations are held ONLY on the dates listed on the form.
